There is no sign of an RAIB investigation of this new one yet, but a lot of relevant material can be found in the report for the earlier one (https://assets.publishing.service.gov.uk/government/uploads/system/uploads/attachment_data/file/996839/140728_R162014_Holland_Park.pdf).
