Title: Double use of ticket, or genuine mistake?

Nice guy sitting opposite me in the South West train headed out from Waterloo.  A ticket check and he hands over his ticket.  "It got punched yesterday in error" he says.  "Oh well - I'll punch it again" says the conductor.  And I found myself wondering - genuine case , or a ticket being used twice for the same journey.  Is it easy for the conductor to tell?

i had the same issue before, and the conductor simply wrote on it stating this from the previous journey etc

The previous conductor should have endorsed the ticket.
