Agreed. GWR seem pretty good at this kind of thing. Maybe XC^▸ is less good because they don't run their own stations and so have fewer staff "on the ground".

Keeps happening...............albeit an overall boarding rather than a toilet issue.

I'd have had that person ejected from the train frankly (she mentioned Virgin staff, assuming this then was one of their services(?), involving the BTP^▸ if necessary to get TGT on too.
