As older people my wife and I have senior railcards and I have found an apparent bug in the way the GWR (Great Western Railway) booking system handles multiple rail cards which results in the traveller being overcharged.

If I select a journey for 2 adults and specify 2 Senior Railcards then the planner shows 2 railcards but only gives me 1 discount.  If I then amend the search I can reduce the number of Senior railcards to 1 and add another line of 1 Senior railcard.  The planner now shows the correct double discount.  If I again amend the search the system has consolidated the 2 instances of the Senior railcard into 1 line with a quantity of 2.

I wonder how many people are not getting the full amount of railcard discount.

Looking at Yatton to Paddington return at 9:40 on 9 March.  After selecting 2 adults, senior railcard x 2 I get the offer of Super Off Peak at £111.40.
If I amend the search, reduce the number of rail cards to 1 and use the + button to get a new line in which I insert Senior Railcard x 1 (Giving 2 lines, each of 1 Senior railcard).
The search now shows the cost of the Super Off Peak as £88.60.  The full price of this ticket for 2 adults is £134.20.

Note that I had set the ralicards in the entry dialogue, and it worked. If I go into the amend dialogue, delete the railcards and the enter two there, I get what you describe - just one discount. that dialogue and the results page don't show how many cards they are allowing, so only the price tells you that.

If I amend an existing railcard entry to two, it works properly. Not only by selecting one, then two, but also by selecting three - which is rejected - and then two. So it appears to be a fault in that dialogue.
