Disabled companion goes free?

I can't seem to find anything anywhere official, or whether the driver was being generous.
Today my wife went on an outing with her wheelchair bound friend, the First bus driver issued my wife a zero fare return ticket, and told her it was due to her being carer for her wheelchair friend.
She says she has always had to pay in the past.
Is this correct procedure that she travels free as companion, or were the previous drivers charging her correct to charge the fare?

As a Disabled Railcard holder myself, I'm aware of the benefits on trains - they're detailed below. In a nutshell, an accompanying adult also gets 1/3rd off. Not sure if this applies to buses as well. It sounds to me like the driver was on this occasion being generous.

If I recall correctly, when I applied for my Wales Concessionary Travel Pass (disabled bus pass) there were two options. The one I have is just for me, the other option was a +1 card where you have a carer traveling with you who also gets to travel free. Might not be remembering that correctly though, and I think the +1 card is not valid if travelling alone (without a carer). It might also be different in England.

Don't DOO^▸ it, keep the guard (but it probably wouldn't be a bad idea if the driver unlocked the doors on arrival at calling points).

It depends on the council, for example Devon CC issue companion passes but Plymouth and Torbay don't (or at least that was the situation over a year ago).
