I did, and I ended up getting charged for the journey twice, but could not get a refund as the total of the unused tickets were under ^10.  The next journey I'm making only cost ^4 anyway.

the booking website did not fully load for me when i tried to use it but as i knew the layout i carried on, somehow 2 adult railcard tickets became 8 (out & return x2 x2) separate tickets when i got to the Fast ticket machine.  as i had to use 4 of the tickets the remaining 4's face value came to less than ^10.  Furthermore my receipt  from the FT machine said I paid ^22 odd, but my online account has me down as paying ^17 something.

Did you select the wrong number of tickets when making the booking, or did FGW (First Great Western) make the mistake?  If it's the latter then FGW legally would have to fully refund the ticket(s) that you didn't order, regardless of any administration charges that are specified if you request a refund for a ticket that you no longer require.

When I booked a ticket using this number, it did not take the ^5 off the first time.

So I cancelled the booking and tried again. It worked.
