Five-and-a-half hours with no toilet on the train.  I recognise that a passenger wanting to travel from one end of this service to the other seems unlikely.  However, how is this considered remotely acceptable for 21st Century travel in a "civilised" country?

I don't suppose it is considered acceptable. But if is to literally not be accepted, what form you you propose that could take?

If you were quick you might make use of the loos at Gloucester/Temple Meads/Westbury, but would almost certainly lose your seat during this evolution. One has to wonder what other passengers would make of someone dashing off the train and leaving a bag behind................still, I suppose the train would be held for the bomb squad to come and blow up your luggage - so you would have plenty of time

On long distance journeys with a faulty toilet, it's common for toilet breaks to be arranged at the larger stations en-route, especially somewhere facilities are available on the platform and there is dwell time, so that customers can nip to the loo.  The TM^▸ (Train Manager, or Ticket Machine, or Temple Meads (Bristol), depending on context) would then make sure they wait until they return, even if a slight delay is incurred.

If a toilet is broken, and no replacement set is available en-route then that is realistically the only option.  That or you cancel the train.

Toilet reliability is now much better on IET^▸ (Intercity Express Train - replacement for HSTs (manufactured by Hitachi in Kobe, Japan))'s but remains very poor on those GWR^▸ (Great Western Railway) units that have been upgraded with Universal Access Toilets, such as the Turbo fleet.  The toilet itself isn't usually the problem, but poor quality components on the door mechanism means the doors can jam open, or shut, or not lock properly.  It needs sorting!
